Last year, my team teacher sent home a little survey-type thing to parents asking for general info about their kids. does anyone have something like that that I could use (I work with young kids, so something appropriate to that), or does anyone have questions they think would be good to put on it? I wasn't smart enough to keep a copy from last year. I'm thinking things along the lines of "what are three things that make your child wonderful?" "what are your child's strengths?"

:) On back to school night, the other fifth grade teacher and I hand each parent a questionnaire in an unsealed envelope. We tell them we would like them to completer this questionnaire, but it is not required. Our principal said we must preface that remark. They can answer any questions they want and return it sealed via your child within the next week. The rate of return tends to be better than 85%. There are 10 questions:
1. What are your child's academic strengths?
2. What is one academic area you will like to see improve?
3. How often does your child read at home independently?
4. What sports or clubs, if any, does your child participate?
5. What other hobbies or interests does your child have?
6. What organizational skills, if any, does your child need to work on? (time on task, budgeting time, knowing what books and supplies are needed, returning forms, etc.)
7. Has your child ever slept away from home for one or two nights? (We take an overnight environmental camping trip in the spring.)
8. Does your child prefer to work alone, in a small group, whole class, or doesn't matter?
9. Does your child have any fears or phobias? If so, what are they? What is the best response to calm them down and get them back on track?
10. Are there any other concerns you would like to share with me? If so, please describe them. (food allergies)
