If so, what do you use?
If not, how do you plan? Do you do novel studies? How do you set up your literacy block?

No plan.
I make my own combination of intution, commerical plans, NAEYC suggestions, and ideas from other teachers.
I try to make my room a literacy-rich enviroment, encorperate some literacy skill into most activites, and offer at least on literacy-specific activity per day.

I do have a programme of guided reading where all the books are graded and the reader progresses through them at their own level, this is to meet "bottom up" requirements. Simultaneously I run a own choice selection of book where all "top down" reading skills can be taught. I also have a literacy rich environment which includes online books etc =)
